A clearer New Referral form

The New Referral form now comes in short sections, with a summary beside it that shows the referral as it will read and what's left before you send.

Pick the specialty first. The form then shows that specialty's common reasons and the practices that offer it. On a dental referral, the tooth chart sits with the reason.

Each section shows a check when it's done. The summary lists what's left; select a line to go straight to it. When it reads Ready to send, the button names who the referral goes to.
